Rules and Regulations for Prize Eligibility
1. Prizes will be given away in the following order: a 2012 Volkswagen Passat 2 Year Car Lease from Wyoming Valley Motors, a Hearts on Fire Necklace compliments of Valentine’s Jewelry, an Apple iPad provided by Golden Business Machines, $499 in Free Gas by Pump n Pantry, a 3-Minute Wegmans Shopping Spree, a Suite from Citizens Bank for a Wilkes-Barre/Scranton Penguins Home Game, a $400 Gift Certificate from Mohegan Sun at Pocono Downs, and $400 in Free Product from Procter & Gamble Paper Products Company (select from Charmin, Bounty, Pampers, and Luvs).
2. All prizes are subject to change based on company specifications and needs. Please call United Way for specific details, restrictions, rules, and regulations on these prizes. No cash substitutions.
3. The winner of the car lease will be required to pay any applicable taxes, transfer costs, and any other associated costs relating to the transaction including, but not limited to, federal and state income taxes and necessary insurance. The winner of the 2012 Passat Lease must receive credit approval by VCI in order to qualify. The winner is also responsible for tax and registration.
4. The Wegmans shopping spree will have a 3 minute time limit. The winner (nontransferable) will start in the produce department. The shopping spree winner will be able to take up to $499 in food and paper products only with no more than 20% of merchandise coming from one department.
5. Date is to be determined by Citizens Bank for the winner of a suite to a Wilkes-Barre/Scranton Penguins Home Game.
6. Each employee or individual who contributes a gift totaling between $156 and $259 to United Way of Wyoming Valley, United Way of Susquehanna County, or one of its partner/participating organizations through this campaign will be offered one chance to enter the drawing. Complete Drawing Entry Form and Entry 1 stub.
7. Each employee or individual who contributes a gift totaling between $260 and $506 to United Way of Wyoming Valley, United Way of Susquehanna County, or one of its partner/participating organizations through this campaign will be offered two chances to enter the drawing. Complete Drawing Entry Form and Entry 1 and Entry 2 stubs. Maximum of two chances per person as applicable.
8. Each employee or individual who contributes a gift of at least $507 a year to the United Way of Wyoming Valley, United Way of Susquehanna County, or one of its partner/participating organizations through this campaign will be offered three chances to enter the drawing. Complete Drawing Entry Form and Entry 1, Entry 2, and Entry 3 stubs. Maximum of three chances per person as applicable.
9. Each contributor must complete a United Way pledge card and this official Drawing Entry Form in its entirety. Failure to do so will result in disqualification. Drawing Entry Form must contain the amount of contribution. A copy of the pledge card must also be included for verification.
10. If a company does not use a United Way of Wyoming Valley or a United Way of Susquehanna County pledge card, copies of internal cards or other pledge verification forms must be submitted with all eligible Drawing Entry Forms by the time indicated in #11.
11. All verified entries must be returned to the United Way office no later than noon on Friday, December 2nd to be eligible for the drawing. United Way is not responsible for delayed or misplaced entries.
12. Members of the Staff and Board of Directors of United Way of Wyoming Valley, United Way of Susquehanna County and their immediate families are not eligible.
13. The drawing will be held in December of 2011, with the winners announced the same day. Date to be announced. Winners have until December 31, 2011, to claim their prize. Proper identification will be required. The drawing time and date are subject to change based on completion of the 2011-2012 General Campaign.
14. The winner, by entering his/her name, agrees that United Way may use his/her name and/or photograph for publicity purposes, upon winning, without further notification or compensation.
15. The winners will be required to sign a release and hold harmless agreement in favor of all prize sponsors, both United Ways, and their respective agents and employees.
